Part D Prior Authorization Update

06/04/2008 - Pharmacists can now complete and sign PA forms for nursing home residents who are beneficiaries of six popular Part D plans! Cigna, First Health, United/AARP, Humana, CCRX/Memberhealth, and Aetna have all approved long term care pharmacists to complete and send in the completed prior authorization forms to the appropriate plans. Please make sure the appropriate clinical data for the drug requiring the prior authorization is included on the prior authorization form and attach any additional information you feel helps show why the drug is needed. The pharmacist must designate on the form that the beneficiary is a resident of a long-term care facility and that the pharmacy is a long-term care pharmacy.
